Title: LICENSED CLINICAL SOCIAL WORKER-ONCOLOGY SERVICES

Other Locations: Gathers and assesses info regarding the patient’s physical needs, mental status, family support system, financial resources and available community and governmental resources. Uses assessment to develop a comprehensive case management plan that will address identified needs. Provides details about resources and options available in the community and coordinates service delivery by making referrals to community resources in collaboration with physicians and patient choice. Provides counseling services in individual, group and family settings as needed. Provides factual info based on working understanding to provide psychosocial support and assist the patient/ family in coping with their disease to improve their overall health care management. Monitors patient care across the continuum through follow-up with patients, families and community services.
